Create a questionnaire

Create an evaluation questionnaire for your documents.

Procedure

Create questionnaire button

  1. Access the Questionnaires module
  2. Click on [+ New questionnaire]

Create a questionnaire

  1. Fill in the general information
  2. Add questions
  3. Submit for validation

General information

Number of attemptsAllowed (1, 2, unlimited)
Score displayImmediate or delayed
Time limitOptional (in minutes)

Add questions

  1. Click on [+ Add a question]
  2. Select the type
  3. Enter the question
  4. Add possible answers
  5. Indicate the correct answer(s)
  6. Define the scoring (points)

Multiple choice example

Question: What is the default frequency for document audits?
○ 12 months
○ 24 months
● 36 months (CORRECT)
○ 48 months

Points: 2

Validation

Workflow: Draft → Validation → Activation (available for use)
